Nurse Practitioner Palliative Care | Gentiva Overview

We are looking for a passionate Nurse Practitioner to join our Palliative Care team at Gentiva. You will play a critical role in delivering compassionate, individualized care that upholds dignity and quality of life for patients and families. Responsibilities

Collaborate with the Palliative Medical Director and palliative team members to diagnose, treat and follow up for patients referred to palliative care. Take responsibility for patient palliative care from the initial assessment through discharge, ensuring assessment of disease‑specific changes, functional decline, nutrition/cognition, pain, falls, infections, and caregiver impact. Establish an individualized palliative care plan in consultation with the patient, family, and team. Facilitate appropriate care within the Plan of Care and participate in evening/weekend calls as required. Prescribe medications, including controlled substances, and order and interpret laboratory, diagnostic tests, treatments, and durable medical equipment as within scope. Communicate clinical findings bi‑weekly to the Palliative Medical Director and reassess the care plan regularly. Maintain effective working relationships with supportive care team members such as Social Workers and Spiritual Care Coordinators. Document clinical findings, plans, and interventions in the patient’s medical record in accordance with standards to support practice and reimbursement. Understand and document Palliative Medicine Quality Measures, collection, and reporting. Qualifications

Specialized Knowledge / Skills Excellent leadership, interpersonal, written and verbal communication skills. Independent decision making and self‑motivation. Knowledge of principles of Palliative Medicine, Medicare/Medicaid regulations, and interdisciplinary processes. Meeting mandatory continuing education requirements of Gentiva and the licensing board. Proficiency with point‑of‑care software. Education / Experience Minimum of a Master of Science in Nursing from an accredited school, or equivalent allowed under state law. Minimum two years of Nurse Practitioner experience. Preferred specialty in Family or Adult Medicine. Part B billing experience. Ability to work autonomously in an Advanced Practice Role. Licenses / Certifications / Agreements Valid registered nurse license or equivalent as required by state law. Current Nurse Practitioner certification/license. Collaborative agreement with a supervising physician for palliative care or willingness to enter one with the Palliative Medical Director. Part B Provider certification. Federal and, if required, state DEA license. Preferred ACHPN – certification in Hospice and Palliative Care. Current automobile insurance and valid driver’s license. Benefits
