Valid NJ Social Work License required Masters in Social Work required At least 2 years experience in healthcare, nursing facility preferred Shift Hours
Monday – Friday 8:00 am – 4:00 pm Benefits

Participate as a member of the team to address Social Service sections of MDS and care plans. Maintain social service records. Complete assessments of new and current residents. Assist with discharge planning. Act as a liaison between facility and the community. Maintain accurate records and report on clients’ status. Location
