Base pay range:
$71,100.00/yr - $97,800.00/yr
Job description summary:
We are seeking a dedicated and compassionate Registered Nurse with a Bachelor of Science in Nursing (BSN) to join our team as a Care Manager, Telephonic Nurse 2. In this pivotal role, you will play a crucial part in our Patient Management Program, focusing on individuals with specialized and complex health conditions, including autoimmune disorders, pulmonary diseases, neuromuscular disorders, infectious diseases, cancer(s), and other rare ailments.
As a Care Manager, Telephonic Nurse, your primary responsibilities will include:
Assess and evaluate the conditions of members with an emphasis on medication therapy through telephonic interactions to ensure optimal wellness.
Develop, implement, and monitor comprehensive care plans based on regular assessments.
Provide medication education using advanced telecommunication systems.
Work independently while adhering to established protocols and guidelines.
Schedule and work environment:
Call center based environment with a set daily schedule of inbound and outbound tasks.
Hours: Monday–Friday 8:00 a.m. – 4:30 p.m. EST; Friday rotation for certain duties.
Saturday rotation every 4 weeks.
Holiday rotation on holidays such as Martin Luther King Day, Memorial Day, Juneteenth, 4th of July, Labor Day, the day after Thanksgiving, and New Year’s Day.
Overtime as needed.
Required qualifications:
Bachelor of Science in Nursing (BSN) – mandatory.
3–5 years of clinical acute care experience.
Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action.
Residency in OH, KY, FL, AZ, or TX.
Comprehensive knowledge of Microsoft Office (Word, Excel, Outlook).
Managed care experience.
Strong communication and schedule adherence skills.
Preferred qualifications:
Experience with care management and patient education for adult care.
Auto‑dialer experience.
Bilingual (English/Spanish).
Travel & remote:
Remote position; occasional travel to Humana’s offices for training or meetings may be required.
