Base pay range $43,000.00/yr - $58,000.00/yr
CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) with at least 3 years of direct patient‑facing work experience. You thrive in a fast‑paced environment, are self‑motivated, have impeccable attention to detail, and value the impact you can have on a patient’s healthcare journey.
In this role you will identify resources and coordinate needs for chronic care‑management patients. It’s great for anyone who loves meeting and assisting new patients daily.

Responsibilities
Accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network.
Resolve patients’ questions and create an open dialogue to understand needs.
Identify and coordinate community resources that would benefit patients’ care.
Provide patient education and health literacy on management of chronic conditions.
Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
Ensure timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.
Work primarily on the phone during most of the shift.
Additional Requirements
Active Compact/Multi‑State license (LPN/LVN).
Technical aptitude – Microsoft Office Suite.
Excellent written and verbal communication skills.
Remote Requirements
Active high‑speed Wi‑Fi.
Home office or HIPAA‑compliant workspace.
Plusses
Epic experience.
Bilingual.
Additional single‑state licensures.
Physical Requirements
Sedentary position with long periods of sitting.
Ability to speak clearly and listen attentively by telephone for extended periods.
Ability to understand, process, and take thorough notes in real time on telephone conversations.
