What are we looking for in a Family Partner? As part of the Youth Mobile Outreach team, the Family Partner utilizes their personal experience to provide peer support to parents and caregivers of youth who are experiencing a mental health crisis. The goal of the Youth Mobile Outreach team is to reduce the use of emergency departments, hospitals, and law enforcement or young people experiencing a crisis by offering an alternative for support and access to services in the community when a mental health emergency does occur. The Family Partner responds in the community to assist with stabilizing the crisis and providing immediate support to the family. The primary focus for the Family Partner is to provide support that allows the youth to stay in their current living situation; promote safe behavior at home, school, and in the community; and assist the youth and their families with accessing ongoing services and treatment. This position is best suited to an individual who is ready to use their own experiences to relate to others and is willing to travel locally throughout the workday.
Family Partner Duties
Sharing personal experiences, as appropriate, to build motivation and encourage clients in their treatment
Providing non-judgmental, unconditional support and consistent hope & encouragement to youth and their families
Modeling ways to engage with others to increase social supports, develop meaningful relationships, and access & navigate community resources
Collaborating with each client to develop and implement individualized treatment plans and self-directed recovery action plans
Completing accurate and timely clinical documentation
Participating in the rotation for after-hours on-call coverage
Qualifications
High school diploma or GED
Must be the parent or guardian of a child who has received mental health treatment services, including being involved in the child's treatment planning process
Will require both AACR and CPSST credentials and NPI registration. AACR will be applied for during onboarding. Will need to complete Peer Support Training through HCA. After completion of training, will start applying for CPSST through the DO
Must be eligible for registration as Agency Affiliated Counselor
Must have valid driver's license and vehicle liability insurance
