In this rewarding role, you will deliver personalized, one-on-one occupational therapy services in patients’ homes, helping individuals regain independence, improve functional performance, and safely participate in daily activities within their own living environment, in accordance with best-practice guidelines established by the
American Occupational Therapy Association (AOTA) . With an average caseload of
4–6 patients per day
and flexible scheduling, you will have the autonomy to build meaningful therapeutic relationships while maintaining a healthy work-life balance. About the Role
As a
Home Health Occupational Therapist (OT) , you will provide skilled, patient-centered occupational therapy services in the home setting to individuals recovering from illness, injury, or functional decline. Your focus will be on improving patients’ ability to safely and independently perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), and functional mobility within their own home environment. You will work collaboratively with an interdisciplinary care team—including skilled nursing, physical therapy, speech therapy, physicians, and caregivers—to develop and implement individualized care plans that promote safety, independence, and quality of life. This role offers the opportunity to deliver meaningful, one-on-one care while making a measurable impact on patients and families in the community. Key Responsibilities
Conduct comprehensive occupational therapy evaluations, including functional status, home environment, and equipment needs; develop individualized plans of care in collaboration with patients, caregivers, and the interdisciplinary team. Deliver skilled occupational therapy services as ordered by the physician and outlined in the plan of care. Establish measurable goals and implement treatment programs to improve or restore function, promote independence in ADLs/IADLs, and teach compensatory strategies when appropriate. Educate and train patients, caregivers, and family members on therapeutic techniques, safety, adaptive strategies, and use of assistive, adaptive, prosthetic, orthotic, and orthotic devices. Communicate patient status, progress, and care needs to physicians, clinical leadership, rehabilitation staff, and other healthcare professionals. Identify needs for additional home health services and initiate appropriate referrals. Supervise and support Certified Occupational Therapy Assistants (COTAs) and Home Health Aides in accordance with organizational policy and state regulations. Participate in care coordination and discharge planning, including community reintegration and transition planning. Recommend and design adaptive equipment and environmental modifications to enhance safety, function, and participation. Administer and interpret functional assessments to support clinical decision-making and physician evaluation. Maintain timely, accurate, and compliant documentation of evaluations, treatments, verbal/interim orders, progress notes, and discharge summaries in accordance with regulatory and organizational standards. Required Qualifications:
Graduate of an
accredited Occupational Therapy program . Active Washington State Occupational Therapist license
in good standing. Current CPR/BLS certification . Excellent
verbal, written, and organizational communication skills . Valid driver’s license , current auto insurance, and
reliable transportation
for patient visits. Preferred Qualifications:
One (1) year or more
of professional occupational therapy experience. Home health experience
and familiarity with in-home care delivery. Experience in home health, acute care, or rehabilitation settings. Familiarity with electronic medical record (EMR) systems. Bilingual skills (Spanish, Russian, Vietnamese, or other languages spoken in our community). Compensation & Benefits

Pay : $45.00 - $70.00 pay per hour/unit, depending on experience (DEO). Earning Potential:
$93,600 - $145,600+ annually. Payment Schedule:
