Responsibilities
Examining patients and reviewing medical records Creating and managing treatment plans with integrated medical practices Prescribing and reconciling medications Educating patients and families about their healthcare needs Coordinating care with providers and overseeing patient engagement Managing gaps in care and ensuring smooth transitions from facilities Collaborating with team members such as medical assistants, social workers, and specialists Requirements
Valid Nurse Practitioner license (Arizona-specific) Experience in primary, acute, or chronic care Strong communication, ethical standards, and patient engagement skills Ability to work collaboratively in a team environment Work Details
Manage approximately 5 home visits daily, with a panel of 100 members Coordinate care planning and follow-up over a year Position covers the Yavapai East area (e.g., Sedona, Cottonwood, Prescott Valley) Competitive starting salary from $130,000/year plus benefits and bonuses Benefits include:
