Job Summary
As a Care Coordinator RN with CHI Health, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health‑care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Screen for appropriateness of care.
Develop discharge plans.
Ensure timely and appropriate discharges by evaluating post‑discharge needs and arranging necessary services.
Maintain knowledge of community resources and alternate care facilities.
Qualifications & Requirements
Minimum 2 years of acute hospital clinical experience.
ADN or a Master’s degree in Case Management, or Master’s Degree Nursing (TX RN License required).
Registered Nurse (RN) license.
Location The Woodlands Hospital – a primary and secondary care hospital serving North Harris and Montgomery counties. Clinical services include cardiovascular services, diagnostic imaging, women’s services (digital mammography and bone density studies), neurosciences, pediatric care (Newborn and Level II nurseries), surgery, pathology, pulmonary services, and sleep disorders. Clinical affiliations include The University of Texas MD Anderson Cancer Center’s Radiation Treatment Center and Texas Children’s Hospital.
Compensation $43.10 – $64.11 per hour
