Job Summary The Ambulatory Nurse Care Manager (ANCM) facilitates the collaborative interdisciplinary process of care management encompassing assessment, planning, facilitation, care coordination, evaluation appropriate to the scope of licensure. The ANCM advocates for options and services to meet the patient’s and family’s comprehensive health needs through communication and available resources to promote quality cost‑effective outcomes for the identified patient population. The ANCM participates in quality improvement activities, exemplifies professionalism, and promotes a customer‑friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.
Minimum Qualifications
Degrees:
Associate Degree in Nursing
Bachelor of Nursing Degree (Preferred)
Licenses & Certifications:
Certificate: Related specialty certification required within 2.5 years of employment
Basic Life Support: Basic Life Support from a hospital‑based American Heart Association (AHA) or Red Cross approved program
Registered Nurse
Work Experience:
One (1) Year of Work Experience: with Care Management, Quality Management, Coding or Discharge Planning (Preferred)
Two (2) Years of Work Experience: as RN in appropriate area assigned
Management Experience: One (1) Year of Management Experience (Preferred)
Language Skills: Spanish (preferred)
Communication Skills
Above Average Verbal Communication (Heavy Public Contact)
