Role We are seeking a skilled
RN or LPN
with MDS experience to lead resident assessments and oversee accurate, up-to-date care plans. In this role, you’ll maintain precise documentation, ensure compliance with state and federal regulations, and collaborate closely with residents, families, and the interdisciplinary team.

Qualifications (RN or LPN)
Active Illinois RN or LPN license
Ability to read, write, and speak English fluently
AANAC certification preferred
MDS experience required
Strong knowledge of nursing services, geriatric care, rehabilitation, and MDS documentation (EMR systems)
Excellent organizational, planning, and time‑management skills
Ability to develop, update, and evaluate care plans
Experience training staff on care plans, documentation, and EMR usage
Ability to audit EMR records for accuracy and compliance
Strong interpersonal communication skills with residents, families, and staff
Compassionate, detail‑oriented, and committed to high‑quality resident care
Familiarity with JCAHO, OBRA, IDPH, and HFS documentation standards
Key Responsibilities
Complete and coordinate MDS assessments for admissions, readmissions, quarterly/annual updates, and significant changes.
Maintain a monthly calendar for MDS assessments & Care Plan Conferences.
Ensure timely completion and accuracy of all MDS sections.
Identify MDS‑related issues and re‑educate staff as needed.
Review and confirm diagnosis sequencing and accurate ICD‑9/ICD‑10 coding.
Lead weekly MDS Pre‑Planning and Medicare meetings.
Support and train staff on EMR documentation and care plan implementation.
Communicate care plans and progress with residents, families, and staff.
Conduct quarterly in‑services on care plan procedures and documentation.
Audit EMR charting monthly and ensure compliance with regulatory standards.
Collaborate with the Director of Nursing and departmental leaders to ensure consistent, high‑quality resident care.
Coordinate care planning with Physical, Occupational, Respiratory, and Speech Therapy teams.
Ensure proper nutritional assessments with dietary consultants.
Maintain confidentiality and follow all safety, infection control, and emergency procedures.
