As a key member of our wound care team, you will:

Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities

Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities

Lead wound assessments, treatment planning, and advanced procedures

Harness cutting-edge tech and evidence-based protocols to accelerate healing

Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families

Keep your charting sharp and timely in our customized EMR

Help drive quality initiatives and support our mission of raising the bar in post-acute care

Qualifications:

  • Active Pennsylvania Nurse Practitioner license (board-eligible not accepted) or Physician Assistant
  • Prescriptive Authority and DEA license must obtain by end of training period
  • At least 2 years of wound care experience
  • Previous experience in SNFs, long-term care, or rehabilitation facilities preferred
  • Willingness to travel to designated partner facilities in the Cumberland/Franklin County areas
  • Comfortable with EMR, mobile, and laptop documentation
  • Strong interpersonal skills collaborative, communicative, and adaptable
