This rewarding role requires a commitment of two days per week to deliver high-quality primary healthcare services within a skilled nursing facility (SNF).
You will be instrumental in managing complex patient cases and driving continuous quality improvement for vulnerable post-acute populations.
Key Roles and Responsibilities Conduct primary care rounds within the skilled nursing facility.
Diagnose medical conditions, manage chronic diseases, and develop comprehensive, patient-centered care plans.
Collaborate closely with nurse practitioners (NPs), facility leadership, and other healthcare professionals to optimize patient outcomes.
Qualifications Open to Physicians specializing in Internal Medicine, Family Medicine, Emergency Medicine, Hospital Medicine, or related fields.
Minimum of 1 year of experience in a similar clinical role is required.
Must possess an active and unrestricted Pennsylvania Medical License (or be in the process of obtaining one).
Must have active and unrestricted DEA/Prescriptive Authority (or be in the process of obtaining it).
Experience in post-acute care is beneficial but not mandatory (training and support are provided). xmvmafu
