The Comprehensive Prenatal Health Worker (CPHW)/Medical Assistant provides support services, ongoing orientations, assessments, care plan development, appropriate nutrition, health education and psychosocial interventions and referrals when needed to UCHC’s prenatal patients. Works under the Clinic Manager to maintain efficient patient flow and quality care during clinic hours, as part of the health‑care team including physicians, NP/PA’s and LCSW to provide comprehensive care in a non‑judgmental manner. UCHC is a federally qualified health center providing comprehensive patient‑centered primary care services to low‑income, multi‑ethnic, uninsured and underinsured populations.

Essential Functions

Conduct face‑to‑face interviews with all prenatal patients, screen for presumptive eligibility (PE) and issue PE card if eligible; issue 2nd good‑through extension for patients.

Identify opportunities and facilitate efforts to expand service or increase patient access into programs.

Assure client charts are documented whenever telephone & face‑to‑face contact is made and code all super bills accordingly.

Provide ongoing orientation, assessments, classes and appropriate referrals when needed to prenatal patients.

Prepare all intake packets in English/Spanish and ensure appointments are scheduled; assist patients in completing Medi‑Cal forms and follow‑up on no‑show/missed appointments.

Order all prenatal forms as needed and fax weekly PE enrollment summary forms to the state; fax weekly schedules to Medi‑Cal worker.

Advocate for patients with Medi‑Cal worker.

Case‑manage the prenatal program: review all prenatal charts after each clinic visit with the provider and review each chart the day before the visit.

Attend recommended job‑related training provided by the CPSP County program.

Perform vital signs and record them in eCW; triage and room patients for telehealth and in‑person visits.

Provide backup support for the medical assistant team; assist training of medical assistants and volunteer staff.

Assist with daily and monthly QA reviews including emergency kit check‑off.

Record and maintain refrigerator and freezer temperature log; perform inventory and order VFC vaccine; order laboratory and medical supplies; check monthly supplies and medications for expiration.

Non‑Essential Functions

As directed by the supervisor, perform other related and/or necessary tasks.

Qualifications

High school diploma or GED required.

Medical Assistant (MA) certificate preferred.

Required: 1‑year experience in a community health center setting.

Knowledge, Skills, and Abilities

Willing to work flexible hours, including evenings and/or weekends; must have reliable transportation and valid CA driver’s license with an insurable DMV record.

Must be self‑directed and work productively with minimal supervision.

Good communication skills necessary.

Bilingual – fluent and literate in Spanish.

Physical Requirements and Environmental Conditions

Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions.

Push and pull objects up to 25 lbs; frequent wrist, hand, and finger dexterity for fine motor function without tremor.

Full range of body motion including twisting, pushing, pulling.

Position requires light to moderate work with ≤25 lbs for lifting and carrying.

Reaching, bending, stooping, handling objects; talking, hearing, seeing.

Working Conditions

Non‑ionizing radiation (microwaves, sun).

Hazardous exposure (chemical, infectious).

Work Schedule

Monday – Friday: 9am – 6pm.

Occasional evenings and Saturdays as needed.

May be traveling to all Unicare Health Center clinics.
