Summary This position assumes responsibility for working together as a team to process and prep all inpatient, outpatient, ambulatory surgery, ER records and loose reports. This position also is responsible for scanning both single documents and batch scanning of any patient’s documentation. The position does quality checks and validation of any and all records for the correct documentation in the correct patient’s record before the record is entered into the EMR. This position is also responsible for keeping and maintaining a productivity log, answering the phone and assisting when and where needed in the HIM department.

Responsibilities

(45%) Prepping/scans all inpatient, outpatient, ambulatory surgery, ER records and loose reports.

(40%) Quality check and validation of all inpatient, outpatient, ambulatory surgery, ER records and loose reports.

(15%) Answering the phone, getting the tube and assisting with other clerical duties as needed.

Other Duties and Responsibilities

Assures quality of all inpatient medical records by prepping the chart in correct order.

Scans the chart into Kofax; quality checks the chart for correct patient documentation.

Validates the chart for correct patient information; answers telephone and responds to phone requests.

Gets the tube and delivers the documentation to the correct location needed.

Keeps and maintains a productivity log.

Relieves coworkers for lunch as needed.

Certifications and Licensures FINGERPRIN - Fingerprinting Required

Required Education High school diploma or GED is required.

Experience, Training and Skills

Organizational skills needed to maintain files.

Ability to make decisions and pay attention to detail.

Interpersonal skills needed to communicate successfully with individuals and groups and interact with employees at all levels within the medical center.

Mandatory OSHA training must be completed annually according to the Exposure Control Plan.

Physical/Mental Requirements and Typical Working Conditions Exposure to stressful situations, including those involving public contact, as well as trauma, grief and death. Able to wear personal protective equipment that includes latex materials or an appropriate substitute if required for the position. Must move freely around the facility with or without an assisted device and perform the functions of the job as outlined in the job description. Overall vision and hearing are necessary with or without assisted devices. Frequently required to sit, stand or walk for long periods. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood‑borne pathogens and other potentially infectious material. Must follow MyMichigan Health blood‑borne pathogen and TB testing as required. Ability to handle multiple tasks, cooperate, work independently, maintain regular and predictable attendance and stay alert. Overall dexterity is required, including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Sedentary. Must occasionally (0‑33% of the workday) lift or carry 0‑10 lbs.
