Social Services Assistant – Canyon Vista Post‑Acute
Join us in providing compassionate care and social services to residents of Canyon Vista Post‑Acute. As a Social Services Assistant, you will play a key role in planning, implementing, and evaluating our social service programs to meet each resident’s emotional and social needs.
General Purpose Assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs in accordance with federal, state, and local standards, as well as our policies and procedures, to ensure that medically related social and emotional needs of residents are met on an individual basis.
Responsibilities
Assist the Social Service Director/Consultant in program planning and execution.
Meet with administration, medical and nursing staff, and related departments to coordinate social services.
Develop, administer, and update department policies and procedures, reviewing them at least annually.
Create and implement policies for identifying residents’ medically related social and emotional needs.
Participate in community planning to advocate for resident and family services.
Help residents achieve the highest practicable level of self‑care, independence, and well‑being.
Provide medically related social services to support residents’ physical, mental, and psychosocial well‑being.
Perform administrative tasks such as completing required forms, reports, and submissions to the Director.
Provide information to residents/families regarding Medicare/Medicaid and other financial assistance programs.
Consult with staff and community agencies to solve resident needs and problems through service programs.
Record and maintain regular social service progress notes reflecting response to treatment plans.
Coordinate social service activities across departments.
Address emotional problems, assisting residents and families with anxieties, stress, and coping difficulties.
Evaluate social and family information to support treatment planning.
Assist in obtaining resources from community social, health, and welfare agencies.
Schedule outpatient appointments and on‑site ancillary services (optometry, podiatry, dentistry, psychiatric services).
Assist with discharge planning and referrals to agency services and equipment.
Develop and maintain rapport with internal departments and external agencies.
Support orientation and in‑service training on social, emotional, and medical needs.
Assist in developing the department budget.
Report suspected fraud or unauthorized disclosure of patient information.
Maintain confidentiality of resident care information, including PHI.
Supervisory Responsibilities This position has no supervisory responsibilities.
Qualifications Education and Experience:
High school diploma or equivalent required. Associate or Bachelor’s degree preferred.
Skills and Abilities:
Read, analyze, and interpret business and professional literature.
Write reports, business correspondence, and procedure manuals.
Present information effectively and respond to questions from managers and employees.
Apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
Solve practical problems in varied, non‑standardized situations.
Interpret written, oral, diagrammatic, or scheduled instructions.
Physical Demands May need to lift and/or move up to 25 pounds. Requires frequent use of a desktop/laptop computer, copying/screening/phone, and occasional emergency assistance for resident evacuation.
Work Environment Typical noise level is low to moderate.
