Join Our Team as a Palliative Nurse Practitioner Hospice Care
Are you a dedicated Nurse Practitioner with a passion for providing compassionate hospice and palliative care? We are seeking a highly skilled Palliative Nurse Practitioner to join our hospice team, supporting patients across Fresno and surrounding areas. This part-time, flexible role offers an opportunity to make a meaningful difference in patients' quality of life while enjoying a collaborative and supportive work environment.
In this role, you will assist the Medical Director with face-to-face evaluations necessary for Medicare recertification, communicate effectively with case managers regarding patient follow-up needs, and provide essential home visits to patients in SNFs, care facilities, or private homes. You may also have the chance to round in inpatient units and educate clinical team members, enhancing overall patient care. This position offers the flexibility of full or half-day schedules, accommodating your availability.
Required Skills:
  • Active Nurse Practitioner (NP) license
  • Palliative care experience
  • Pediatric (PEDS) experience preferred
  • Strong communication and interpersonal skills
  • Ability to perform face-to-face patient evaluations
  • Flexibility to travel within the Fresno area
  • Basic understanding of Medicare regulations and recertification processes

Nice to Have Skills:
  • Experience working in hospice or home-based care settings
  • Prior experience in inpatient rounding or home visits
  • Bilingual in English and Spanish
  • Knowledge of care coordination and interdisciplinary team collaboration

Preferred Education and Experience:
  • Master s degree or higher in Nursing
  • Minimum of 2 years of Palliative or Hospice care experience
  • Pediatric care experience is highly valued

Other Requirements:
  • Valid driver s license and reliable transportation
  • Ability to work independently and adapt to a flexible schedule
  • Compliance with all licensure, certification, and travel reimbursement policies
