About Fort Logan The Colorado Mental Health Hospital in Fort Logan (CMHHIFL) is one of two state hospitals serving our community. Its vision is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness. The mission is to support individuals in achieving recovery goals and reaching their full potential. CMHHIFL provides inpatient treatment to adult patients, generally between the ages of 18‑59, and has 138 adult inpatient beds across six treatment milieus serving civil and forensic individuals.

Position Overview The Social Work III collaborates with patients, families, support systems, and external stakeholders to plan, organize, and execute psychiatric treatment interventions and comprehensive after‑care plans. Responsibilities include clinical assessment, treatment planning, individual, group, and family therapy, liaison with Community Mental Health Centers, coordination of discharge and after‑care, and contribution to hospital‑wide events and training.
Responsibilities
Provide individual, group, and family therapy as determined by treatment plans and team collaboration.
Develop group materials appropriate for patient needs and lead group therapy sessions.
Complete comprehensive biopsychosocial assessments and reassessments, documenting in the electronic health record.
Coordinate treatment planning meetings, ensuring goals are SMART and compliant with regulatory requirements.
Manage discharge planning, including benefits submission, guardianship needs, Momentum referrals, Medicaid/Medicare application, and placement coordination.
Support implementation of specialty programs such as Animal Assisted Therapy, Substance Addiction Treatment, Virtual Reality programs, and Peer Programs.
Supervise social work externs and support their professional development.
Contribute to hospital‑wide initiatives, in‑service training, and program development.
Minimum Qualifications
Current, valid Colorado licensure in any of the following: LSW, LCSW, LPC, LMFT, LAC.
At least one year of licensed experience in a role consistent with the responsibilities of this position.
Resident of Colorado.
Preferred Qualifications
Social work degree.
Inpatient psychiatric treatment experience.
Experience with adults with severe and persistent mental illness.
Forensic and/or competency restoration program experience.
Former State employee.
Conditions of Employment
Full background checks including CBI & FBI fingerprint, Colorado Court Database, Medicare Fraud Database, TRAILS Database.
Motor Vehicle Check if driving a state‑owned vehicle.
Valid, non‑restricted Colorado Driver’s License (or U.S. Driver’s License for locations within 30 miles of the border).
Age at least 21 years.
Ability to handle hazardous materials, including bodily fluids.
May need to travel for training, court hearings, patient transport.
TB testing, annual influenza, and other vaccines as required by the State of Colorado.
