Social Worker Home Care
MedStar Health is looking for a Social Worker Master of Social Work (MSW) Per Visit to join our Home Care team. The ideal candidate will be a licensed Social Worker with strong clinical and communication skills, providing counseling and linkage to community services in line with home health regulations and Montgomery County contract requirements.
Responsibilities
Document care provision according to state, federal regulations and agency policy, and communicate verbally and in writing with other care providers.
In collaboration with the healthcare team, patient, and family, develop and initiate a social care plan based on assessed needs; continually reevaluate and revise the plan as needed.
Maintain effective working relationships with other departments and participate in multidisciplinary quality and service improvement teams.
Perform appropriate actions to meet patient needs, link with community resources, provide short‑term counseling, assist with nursing home placement, and support staff in addressing patients’ social and emotional needs.
Qualifications
Master’s degree in social work from a school accredited by the Council on Social Work.
1–2 years of social work experience in a healthcare facility or setting.
Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), Licensed Certified Social Worker Clinical (LCSW‑C), Licensed Independent Social Worker (LISW), or Licensed Independent Clinical Social Worker (LICSW).
CPR certification.
Valid State Driver’s License in good standing.
Home Care experience preferred.
Salary Range $60,632.00 – $101,899.20 per year.
