Overview
Wound Nurse (Part-Time) at OPCO Skilled Management. Treatment Nurse / Wound Nurse. Reports to: Director of Nursing and/or ADON. Supervises: None. Classification: Non-Exempt Hourly. Position focuses on the skin management program and delivering safe, efficient treatment for wounds, including surgical openings, ulcers, bed sores, feeding tube sites, and abscesses. Requires wound assessment, development and implementation of treatment plans, infection monitoring, wound cleaning, and adherence to the state Nurse Practice Act. Principal Responsibilities
Perform resident skin assessments, obtain treatment orders, administer treatments as ordered, and document to improve and/or maintain skin integrity. Administer treatments prescribed within standard nursing practice guidelines and in compliance with infection control protocols. Coordinate and lead weekly wound rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation. Document findings, contact physician with recommendations for new or revised treatment orders, monitor wound healing progress, and adjust as appropriate. Receive, transcribe and implement new treatment orders promptly. Collect, analyze and provide pressure ulcer information to nursing staff and other nursing personnel. Maintain records of pressure ulcers and skin alterations, including risk assessment, prevention, weekly pressure ulcer assessments and measurement documentation, and calculate weekly/monthly pressure ulcer rates. Investigate center practices and risk-minimizing measures, including Braden risk scales on admission and preventive interventions. Monitor timely implementation of preventive interventions for at-risk residents, including new admissions and residents with skin alterations. Coordinate ongoing pressure ulcer and skin alteration tracking activities and implement measures to improve outcomes, including employee education. Recommend education for employees to minimize or prevent center-acquired skin alterations and pressure ulcers. Educate newly hired staff on Skin Practice Guide requirements and Skin Resource Library, including wound assessments, preventive interventions, care planning, treatment options, and documentation requirements. Collaborate with administrator, medical director or designee, Director of Nursing, and other team members on Skin Practice Guide. Evaluate availability of wound care supplies and coordinate ordering with central supply. Assist the Director of Nursing and medical director with case review for center-acquired pressure ulcers. Monitor changes in state and federal regulations and industry practices; stay current with industry trends in skin and wound management. Provide monthly, quarterly and annual reports with recommendations for changes in center practice to the Quality Assurance and Performance Improvement Committee. Other duties may be assigned as needed. Direct Care Responsibilities
Chart progress notes accurately to reflect care administered and resident response. Follow procedures for charting and reporting incidents/accidents (residents and employees). Report changes in condition to supervisor, physician, and family. Identify skin changes and follow company skin management protocols. Order and report diagnostic tests timely. Communicate resident changes at shift handoffs. Complete documentation reflecting progress or lack of progress of the resident’s skin management. Be familiar with the forms used in the resident’s chart. Participate in investigations related to alleged abuse when appropriate. Ensure progress notes reflect the care plan and that care-plan approaches are followed; handle special nursing care responsibilities (e.g., NG/tube, colostomy, Foley catheter, tube feeding, positioning, transfers, ROM, emergency procedures, IV care, suctioning, ventilator-related care) as applicable and within scope. Attend trainings and in-service sessions to maintain current certification and comply with regulatory requirements. Adhere to work schedules, attendance requirements, and center policies; cooperate with staff to maintain services for residents; maintain a warm, respectful, and calm environment. Adhere to dress code and other employment policies. Customer Service Responsibilities
Model customer service principles and promote understanding of residents’ needs. Treat residents, families, visitors, and staff with courtesy, respect, and dignity. Present a professional image through appearance, behavior, and communication; explain procedures to residents before administering services. Residents Rights
Uphold and promote residents’ rights; ensure they can exercise rights without interference or discrimination. Protect resident health information confidentiality and respond to concerns/complaints with tact and urgency. Report allegations of abuse, neglect and/or misappropriation of resident property. Coordinate interpreters and auxiliary aids to assist residents as needed and ensure appropriate accessibility. Safety and Sanitation
Follow safety policies and procedures; identify and correct safety hazards. Observe safety needs of residents as indicated in care plans; use safety equipment as trained. Demonstrate knowledge of fire and disaster preparedness during drills or actual events. Staff Development
Attend in-service trainings and ensure compliance with company policies; conduct small group meetings as required; communicate new policies and procedures to staff to ensure compliance. Qualifications
Graduate of an approved RN or LPN program and licensed in the state of practice. One-year experience providing specialized treatments in a long-term care setting; minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Current BLS/CPR certification (as required by state for SNFs/ALFs). Local travel to attend trainings and/or workshops. Seniority level
