Position Summary The Clinical Staff Pharmacist is responsible for providing safe, effective, and timely pharmaceutical care to patients in accordance with professional standards and regulatory requirements. This role collaborates closely with physicians, nursing staff, and other healthcare professionals to ensure optimal medication therapy outcomes. The pharmacist also provides education and consultation to patients and families as needed.
Key Responsibilities Dispense prescribed medications and pharmaceutical products accurately and safely Review medication orders for appropriateness, dosing, interactions, and allergies Provide clinical consultation and medication therapy recommendations to medical and nursing staff Counsel patients and families on medication use, side effects, and adherence Ensure compliance with hospital policies, state and federal regulations, and accreditation standards Participate in interdisciplinary rounds and clinical initiatives as assigned Maintain accurate documentation within the pharmacy and electronic medical record systems Support quality improvement and patient safety initiatives
Qualifications Required: Doctor of Pharmacy (PharmD) from an accredited program Active pharmacist license (or eligibility for licensure, per facility requirements)
Preferred: Minimum of
1 year of post-graduate hospital pharmacy experience
