Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with an emphasis on serving the Chicano/Latino/Indígena community in San Francisco with a special focus on the diverse needs of the Mission District. Services include a wide range of mental health, HIV-related services, and social services, including health promotion, education, prevention, early intervention, case management, and psychological and psychiatric interventions, as well as cultural/social and spiritual re-enforcement. The agency serves children, youth, adults, and families. It has a rich history of working collaboratively with other agencies to meet the needs of the Chicano/Latino/Indígena community and other cultural/racial communities in San Francisco.

Job/Program Description

The La Cultura Cura Program offers a range of services and opportunities to young people ages 12-24 and their families with a particular emphasis on youth that are; impacted by community and interpersonal violence; need access to mental and emotional support; and/or have had contact with/or are at risk of entering or interfacing with legal and criminal justice systems. La Cultura Cura offers case management, mental health and group services in different community settings and youth participants have access to culturally oriented structured enrichment programs. Please note this position does not provide supervision to others.

This position requires someone who can demonstrate cultural competency in addressing the diversity of the community, including: gender, race, language, sexual orientation and socio-economic status. The ideal candidate for this position is a highly motivated and community-driven bilingual (English/Spanish) professional, who has a passion for youth development work, has an understanding of community resources available in San Francisco and the surrounding Bay Area, and has experience working in violence prevention, restorative practices and knowledge of legal justice systems, as well as behavioral health systems.

Essential Job Duties are, but are not limited to the following:

Individual Case Management:

Manage a caseload of a minimum of 8 youth at a time, and 12 - 16 youth per year as assigned. Provide intensive and restorative care management, access to cultural affirmation activities, and family engagement services to youth and young adult - with an emphasis on serving newcomer immigrant youth and clients stepping down from higher levels of clinical behavioral care. Collaborate with Mental Health Specialist to complete Initial Assessment (CANS/ANSA) and Care Plans within 45 days of episode opening in order to identify needs, risks, strengths and interventions to support youth engaged in LCC Mental Health Services. Schedule at least one weekly appointment with each individual participant to review care plan goals, implement interventions and complete linkages and referrals (In-person and remotely). Connect individual participants and their families to resources by linking them to access points available in San Francisco (i.e, educational & legal, basic needs, PPE, technology, housing support, health services, and peer socialization activities). Coordinate care with Comprehensive Child Crisis and hospital staff to ensure appropriate level of care for youth stepping down from higher levels of care. In support of participants in Juvenile and Adult Justice Systems, CM will attend court proceedings, Probation Officer, and Public Defender meetings and will coordinate efforts with the youth's carrying probation officer to ensure youth is supported in meeting legal obligations related to their release orders and to advance community service requirements. Take the lead position in communication with each member of participant’s interdisciplinary team of service providers and caregivers by coordinating care with other providers, caregivers and by convening Child Family Team conferences to ensure service continuity and elimination of gaps. Conduct school, home and community visits, including telehealth services when appropriate and pursuant to IFR, CDC and SFDPH policies and procedures.

Workshops/Groups

Co-Develops & Implements curriculum for two 10-week school-based psycho-education/cultural affirmation groups focused on supporting youth to develop a better understanding of trauma, to support them in strengthening their understanding of healing practices, and to increase their life skills and social engagement/connection, utilizing multiple clinical and culturally relevant traditional & indigenous interventions. Will support efforts to engage LCC participants in healthy peer socialization activities during Summer Programming.

Documentation & Other Tasks

Complete Intake, Psychosocial Assessment and Care Plan for each individual youth participant on case load, in order to determine service level need. Maintains hard and electronic documentation, including medical health records, service track logs, and client records for system statistical data processes and meets standards of Quality Assurance and HIPAA. Complete billing sheets and monthly reports to the Program Manager/Director and to funding sources. Participate in collaborative meetings, intra-agency trainings and meetings, violence prevention activities, and other events as requested. Support the LCC team with Summer Programming efforts. Participate in designated outreach activities. Perform other duties as assigned.

REQUIRED SKILLS:

Bilingual English-Spanish. At least 2 years of experience providing services to youth ages 12-24 experiencing mental health needs. Knowledge of case management practices. Demonstrated cultural competency in addressing the diversity of the Latino community including gender, race, language, sexual orientation, and socio-economic status. Good written and oral skills. Ability to work a flexible schedule at multiple locations. At times, this position requires at least one evening shift and extensive travel between different locations. Experience with collaborative work and care coordination.

EDUCATION, CERTIFICATION AND LICENSES

BA Degree in Social Work or in a related field or any combination of experience and/or education equivalent

CONDITIONS OF CONTINUED EMPLOYMENT

Must pass and maintain annual Tuberculosis Test Must pass the Department of Justice Fingerprint screening Complete annual recurrent online training

Compensation And Benefits²

Salary Range:

This is a

temporary

full-time position

(until June 30, 2026)

with benefits, with an hourly salary range of $27.08 to $31.00.

Commensurate with work experience, qualifications, and education.

All full-time positions are based on a minimum of thirty-five (35) hours per week.

This is a temporary full-time non-exempt position at 35 hours a week (until June 30, 2026).

The position is eligible to participate in medical, dental, vision, and 403(b) plans, earn vacation, sick leave, paid holidays, two wellness days (birthday and hire date) and receive ongoing opportunities for learning.
