Longwood Hospice’s Mission is to provide compassionate, holistic end-of-life care, centered on a personal approach, connectedness, dignity, and clinical excellence. Focus of care is directed toward patient goals for the remainder of life and is adjusted and refocused as the condition of the patient changes. The Community Liaison is responsible for acting as the primary conduit between Longwood Hospice and physicians, acute care, hospitals, skilled nursing centers, assisted living facilities, and other community-based referral sources. May also conduct personal visits to primary referral sources and interested parties and represent Longwood Hospice in community events. Essential Job Functions and Duties:

Identify and develop ongoing contacts with physicians, specific acute care hospitals, skilled nursing centers, assisted living facilities, and other community-based referral sources to maintain relationships for the promotion of public awareness and understanding of the hospice program and philosophy. Provide information to physicians, families, and other referral sources to promote awareness for end-of-life care. Serve as the liaison between community-based referral sources and the Longwood interdisciplinary team. Participate in regularly scheduled IDT meetings to coordinate the care of the individual and family, exchange information and solve problems, and receive staff support and in-service information. Coordinate the admission process including assignment of Longwood team members to conduct the initial assessment and ongoing communication with the referral source as necessary. Complete all referrals and intake admission paperwork in a complete, accurate, and timely manner. Provide in‑services to community agencies, physicians, and hospital referral sources. Educate Longwood team members in customer service aspects as needed. Participate in Longwood’s Quality Assurance/Performance Improvement program as requested. Apply policies and procedures of Longwood and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in performing duties. Maintain all individual privacy and confidentiality in accordance with Longwood policies and HIPAA requirements. Ensure that all care is provided consistently with respect for individual rights. Report any/all allegations of individual abuse, neglect, and/or misappropriation of property in accordance with Longwood policies. Maintain required clinical competency/license as appropriate. Follow standard precautions and infection control procedures at all times. Attend and participate in all in‑service training and meetings as needed. Participate in developing and updating Longwood policies and procedures as requested. Report all accidents and incidents observed. Identify and respond appropriately to emergency situations. Complete other duties as assigned. Position Qualifications and Entrance Requirements:

Bachelor’s degree (BA or BS) from a 4‑year accredited college or university. Must have at least 2 years of sales and marketing experience. Must have demonstratable competency with computers (MS Word, MS Excel, etc.). Solid verbal, written, and interpersonal communication skills are required. Experience working independently with minimal supervision is required. Candidates must successfully complete criminal background checks and physical (including TB screening) prior to completing Orientation. Preferred skills and/or experience:

Previous hospice and/or healthcare sales experience is preferred. Experience in a customer‑service oriented environment is preferred. Physical Demands/Work Environment:

Must be able to complete physical activities (walking, standing, talking, hearing, pushing, reaching, seeing, stooping/crouching, smelling, depth perception, identifying colors) as needed. Must be able to travel to individual locations daily. Must be able to work with exposure to possibly disagreeable conditions including but not limited to: odors, infection, illness, and physical contact with disruptive individuals.
