Discharge Planning Assistant Be a key resource for the Care Coordination Team, initiating referrals, coordinating transportation, and providing timely updates on patient care and insurance details.

Responsibilities

Initiate referrals for subacute, acute, psychiatric, homecare, and DME to identified providers.

Print, copy, and fax documentation and files.

Obtain authorizations when appropriate.

Coordinate transportation for patients and act as the point of contact.

Communicate with various providers, facilities, and the patients' families to obtain information about patients' previous placements and plan of care upon discharge.

Provide timely and accurate updates to staff regarding Medicaid or other insurance details that could impact the patient's care.

Call physician offices to schedule follow-up appointments, document in the EMR, endorse to Care Coordination Nurse, and inform the patient when instructed.

Contact community resources such as homeless shelters and drug treatment programs to determine bed availability.

Requirements

Bachelor's Degree in Social Work, Social Sciences, or related field.

Strong interpersonal and communication skills.

Ability to organize and manage multiple time‑sensitive tasks.

Ability to simultaneously cover multiple patient care assignment areas reflecting various ages, populations, and diagnoses.

Hospital experience and direct experience with patients preferred.

Salary & Benefits Salary Range: 21.13 – 34.86 USD (full‑time). The salary reflects full‑time compensation and will be prorated based on employment status.

Competitive base rates are determined by many factors, including job‑related work experience, internal equity, and industry‑specific market data. Some positions may qualify for clinical certification pay and shift differentials.
