Deer Creek Nursing & Rehabilitation is currently searching for a dedicated Social Worker to join our team! Experience with geriatric patients or in a Skilled Nursing Facility is a requirement. Our facilities deliver outstanding care and foster strong employee bonds. We offer short-term to long-term care and a variety of living options and professional clinical services. Social Worker Responsibilities

In consultation with the Administrator, facilitating all admissions and discharges.

Ensure development and regular review of appropriate discharge plans in conjunction with overall plan of care.

Provide family conferences and serve as a support and liaison between the participant, family, and Center.

Facilitate participant’s adaptation to the adult day health care program.

Promote and facilitate participant active involvement in plan of care as appropriate.

Assess participants for signs of mental illness and/or dementia and make appropriate referrals.

Arrange and coordinate additional support services which are not available at the Center.

Serve as a participant advocate by safeguarding their civil and human rights.

Assist in discharge planning, transition and follow‑up.

Participate in social service quality assurance evaluation.

Follow‑up of hospitalized participants.

Facilitate support groups for family and participants through individual or group counseling.

Conduct support groups or facilitate participant or family/caregiver participation in support groups.

Assist program staff in adapting to changes in participant’s behavior.

Social Worker Requirements

LMSW/LSW is required.

Experience with geriatric patients or in a Skilled Nursing Facility is a major plus.

Successful completion of a training and competency evaluation program required.

Completion of orientation required by company policy.

Health screening and TB test (if required) within two weeks of employment.

Mandatory drug test within 90 days of hire.

Must be free of criminal activity proven by a criminal background check.

Show satisfactory evidence of identity and eligibility for employment within three (3) days of hire.
