What You'll Do as a Case Manager
Conduct client assessments and help develop individualized care plans
Coordinate services with therapists, doctors, nurses, and other care providers
Monitor client progress and adjust care plans as needed
Document services accurately and maintain updated case notes
Collaborate with a multidisciplinary team to ensure clients receive comprehensive support
Use creativity, empathy, and problem-solving skills to help clients overcome challenges
Qualifications Applicants must meet
ONE
of the following:
Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.
Associate's degree or equivalent and working toward SSW licensure
Bachelor's degree with Social Service Worker (SSW) license
Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
Valid driver's license
Basic understanding of Social Work principles
Familiarity with social welfare policy and human development
Ability to problem-solve and maintain a client-centered approach
Spanish speakers may qualify for increased wages after testing
