The Mental Health Case Manager is responsible for providing direct support to clients and their families enrolled in outpatient mental health services, including accessing services, outreach/engagement support, service coordination/referrals, case management, and discharge planning. The role works with MH therapists to assess client and family needs and develop a plan to meet those needs, guiding families through complex systems to achieve treatment goals. Duties and Responsibilities
Assess needs of client and family members, identifying risk factors and creating a case management plan that outlines goals/objectives. Provide service coordination, referral, linkage and advocacy for clients and families based on assessed needs. Collaborate with caregivers, schools, hospitals, DHS/CPS, MH providers, wraparound providers, and other entities to ensure client needs are met. Provide client/family outreach and engagement support. Support clients and families during transitions or crisis. Support discharge planning for clients. Develop and maintain resource lists, group therapy options, extracurricular activities, parenting resources, etc. Develop and maintain community and external agency partnerships. Conduct screenings for ancillary services and/or higher levels of care. Provide psycho‑education to clients and families. Complete timely clinical documentation for billable and non‑billable services in accordance with state, federal and organizational requirements. Maintain efficient and effective clinical and administrative operation practices. Adhere to outpatient policies, procedures, and protocols and those outlined in the Morrison Child and Family Services Employee Guidebook. Perform other related duties as required and assigned. Demonstrate commitment to developing a thorough knowledge of the Sanctuary model and other organizational policies & practices. Participate in development, in‑services, and training related to workplace effectiveness; model professional behaviors and implement improvements to business practices with awareness of their impact in a trauma‑informed behavioral healthcare organization. Core Competencies
Understands mental health assessment, treatment, case management, and service terminology. Ability to perform administrative and organizational functions such as managing spreadsheets and reports, entering and tracking data, scanning/uploading files, navigating multiple databases, and maintaining records. Ability to read, record and interpret information including client insurance eligibility and information in Electronic Health Record (EHR) systems. Behaves professionally (demeanor, dress, language, commitments, etc.). Demonstrates the ability to recognize professional limitations and seeks supervision when appropriate. Acknowledges and respects all individuals and demonstrate openness to providing trauma informed responsive care and treatment to clients and their families. Demonstrates knowledge and skill in assessing suicide risk factors and facilitating screening and triage. Demonstrates sufficient knowledge to support clients experiencing a mental health crisis including de‑escalation techniques and facilitating access to appropriate crisis services. Understands and complies with mandatory reporting requirements. Use developmentally appropriate and trauma informed approach to follow agency protocol for assessment and intervention. Required Position Qualifications
A Bachelor’s degree in Behavioral Health or related field OR a combination of at least three years of education and/or experience in behavioral health field. Care coordination or case management experience at a behavioral health or related agency. Knowledge of state and county systems and resources. Preferred Position Qualifications
Bilingual abilities, including the ability to speak, read, write, and translate documents in Spanish. If bilingual in Spanish, must clear a language proficiency test. Other Requirements
QMHA Registration within 30 days of hire. Personal transportation allowed for travel between program sites. Valid driver’s license, proof of personal auto insurance liability coverage, and a driving record that meets corporate driving criteria are required. If driving a personal vehicle for work purposes more than once a week (4 times per month on average) must have and maintain personal auto liability coverage at $100,000/$100,000 level. Must pass criminal history check. Working Conditions – Environment
Work is done in clinical offices, community settings, including Morrison Residential, administration buildings, public locations, and social service sites. Breaks and meals are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise, and contaminants. Work assigned is diverse and may involve addressing new and unique circumstances. The work might be unpredictable and disrupt planned tasks, requiring flexible time management. Working Conditions – Physical Demands
Ability to remain calm and emotionally available serving youth, adults and or families who have experienced trauma. Proficient with office equipment and physical movement. Ability to perform physical interventions if needed in accordance with program and training. Physical dexterity for reaching, stooping, and repetitive wrist and hand motions.
