AristaCare Health Services is seeking a
Social Worker
to join our Green Brook facility. The role involves planning, developing, organizing, implementing, and evaluating social service programs, while enforcing company policies and practices.
Responsibilities
Participate in community planning related to facility interests and resident needs.
Assist in discharge planning, development and implementation of social care plans and resident assessments.
Engage residents and families in planning social service programs when possible.
Coordinate transportation to other facilities as needed.
Refer residents/families to appropriate social service agencies when facility services are insufficient.
Requirements
Associate Degree required.
Must hold LCSW or CSW license.
2-3 years’ experience in assisted living or skill nursing required.
Excellent interpersonal and conflict resolution skills.
