Job Overview We are seeking an experienced and compassionate
Caregiver II
to provide hands‑on personal care and support to patients receiving home care and/or center‑based services. Under the direction of a Supervisor or Manager, this role supports patient dignity, safety, and independence while delivering care in accordance with individualized care plans.
In addition to direct patient care, the Caregiver II plays a
leadership role
by assisting with onboarding and ongoing training of fellow caregivers. This position is ideal for a caregiver who enjoys mentoring others and contributing to a collaborative, patient‑centered healthcare environment.
What You Will Be Doing Provide personal care and escort assistance per individualized care plans, including:
Grooming, dressing, peri‑care, bathing, showering, and toileting
Housekeeping, laundry, meal preparation, and feeding assistance
Observe, document, and report changes in patient condition in a timely and professional manner
Follow safe care practices, including proper body mechanics, universal precautions, and maintaining a safe environment
Interact professionally with patients, families, and care team members, encouraging engagement in conversation and activities
Perform all Caregiver I duties as needed
Support onboarding and ongoing training of Caregiver I and II staff using established training materials
Participate in departmental and family meetings as required
Utilize safety equipment properly for lifting and transfers; model and reinforce safe techniques for others
Perform other related duties as assigned
What You Must Have Experience
Minimum 1 year of experience working with elderly or frail populations
Experience caring for seniors with dementia
Knowledge, Skills & Abilities
Understanding of an integrated, patient‑centered care model
Flexible, positive attitude with the ability to work across multiple care settings
Ability to work independently and collaboratively as part of a team
Strong interpersonal and communication skills
Basic English reading and writing skills required
Basic computer skills, including the ability to use or learn email and electronic health record systems
Ability to explain and demonstrate safe lifting techniques and proper use of safety equipment
Bilingual ability may be required for specific assignments and daily patient interaction
Licenses & Certifications (Required Upon Hire)
Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
CPR (Cardiopulmonary Resuscitation)
First Aid
Nice to Have
Bilingual proficiency in Cantonese or Spanish (in addition to English)
At least 1 year of experience in a home care or clinic setting
Prior experience mentoring or supporting caregiver training
