Part‑Time RN Tele‑Triage (Fully Remote)

Tuesday, Wednesday, Thursday, Friday, 4 p‑9 p CST (5 p‑10 p EST)

Starting Point Nursing Services provides remote registered nurses who support the post‑acute continuum with clinical advice and interventions. The 24/7 On‑Call and Triage team responds to physician practices, home health agencies, intellectual disability providers, and assisted living providers.
Responsibilities
Monitor assigned phone lines during service hours to ensure timely response for caller needs.
Triage all symptom‑based calls and provide recommendations according to the approved triage protocol.
Adequately assess severity of symptoms and refer to appropriate level of care.
Document all care‑related recommendations in real time and notify local operations of triage supports and outcomes.
Coordinate care with acute care settings for clients receiving ED/hospital services, including discharge planning and medication orders.
Coordinate medication orders with assigned pharmacy and update MAR/QuickMar.
Complete and conduct assessment and reassessment of persons served.
Develop and update care plans, secure physician orders as needed.
Communicate with beneficiaries via phone, text, web chat, or email.
Ensure compliance with state/local/payer requirements through documentation review.
Provide coaching and education on health and well‑being of persons served.
Participate in performance improvement activities and maintain clinical knowledge through training.
Document actions in Nurse’s Notes or Service Delivery Log.
Maintain a time‑sheet log for weekly submission to supervisor.
Notify practices, physicians, and managed‑care clients of all encounters with patients.
Notify leadership of operational concerns and actively participate in meetings and committees (e.g., IDT, HRC, Safety/Quality).
Qualifications
Accredited nursing degree or certification.
Current Registered Nurse license in the state of practice; must hold a compact RN license.
Clinical experience providing services to individuals with intellectual and developmental disabilities, home health, hospice, or related disorders.
Two years of Home Health nursing experience.
Remote Triage & Home Health RN experience highly preferred.
Compensation USD $33.00 – $35.00 per hour.
Additional Information Potential start date: January 19 2026. Immediate opening: one part‑time position.
