We want you to grow your career with us!***Job Summary:**The primary function is to process all aspects of scheduling nursing visits via Home Care Home Base software system.**Functions:*** Schedule and appropriately document patient schedules in a timely manner.* Participate in coordinating care with management and patient interdisciplinary team.* Provide effective communication to patients, staff members, other health care professionals and referral sources.* Respond to agency and patient needs in a professional and creative manner.* Demonstrate commitment, professional growth and competency.* Promote Agency philosophy and administrative policies to ensure quality of care.**Skills & Experience:*** **Minimum 1 year of home health experience.*** Excellent computer skills required.* Excellent interpersonal and organizational skills.* Basic knowledge of scanner/fax/copier equipment.* Phone etiquette skills required.* Ability to take direction with a multitude of tasks.* Typing ability 40-50 WPM.* Knowledge of Medicare regulatory guidelines.**Qualifications:*** **Graduate of an accredited school of practical/vocational nursing.*** **Current state license as an LVN.*** **Current State’s Driver’s License.*****A reliable source of transportation and valid/current auto liability insurance is required.****We are an equal opportunity employer.
