Evening Visit RN

is primarily responsible for planning, organizing and directing home care services after hours. This clinician is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

for Home Health

RN Hourly Range:

$42.00 - 45.00 hourly, depending on experience PLUS pay-per-visit stipend of $75/per complete visit

Service Area:

Metro Area

(Minneapolis, St. Paul and surrounding).

Plus, mileage reimbursement of .70 cents per mile.

Schedule: Part-Time (3-4 days/week), 5pm-11pm with every 3rd weekend rotation and 2 holiday rotations per year (1 winter/1 summer)

Position Functions & Responsibilities:

Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).

Uses health assessment data to determine nursing diagnosis.

Assess the problem based on conversation with client/patient, family or caregiver

Uses clinical judgment, knowledge and experience to direct conversation and guide decision-making to give self-care advice, refer client/patient to physician, make an appointment, instruct client/patient contact emergency assistance or complete a skilled nurse visit.

Provides direct/triage client/patient care as

Honors Client Confidentiality, Rights, Privacy, and Reporting Maltreatment Expectations.

Understands and Accepts Clients’ Diagnosis.

Understands and accepts emotional needs of client.

Accepts unique client symptoms and behaviors.

Understanding of Confidentiality & Data Protection Act.

Adheres to Emergency Procedure and Response Expectations

Understands and complies with all rules and regulations that govern skilled nurse care responsibilities.

Prioritizes skilled needs after hours with effective triage and ensures client needs are met and scheduled effectively

Completes accurate documentation with the EMR

Identifies home safety issues and notify appropriate care team members or leadership

Demonstrates proper procedures: handling, lifting, transfers.

Participates in Lifespark event management process including initiation of reports, escalation of events, and investigation

Qualifications:

One year of home health care experience preferred

Recent triage experience in clinic, ALF, SNF, or Home Health

Strong clinical skills

Graduate of an accredited school of nursing

Participate in weekend and holiday rotations

Must pass a MVR check based on Lifespark company policy
