Job Summary The Case Worker at Sts. Joachim and Ann Care Service provides compassionate, client-centered support to individuals and families in need. This role focuses on assessing client needs, developing and coordinating service plans, advocating for resources, and promoting stability, dignity, and overall well-being. The Case Worker works collaboratively with internal teams, community partners, and service providers while upholding the mission and values of Sts. Joachim and Ann Care Service.

Job Responsibilities

Conduct comprehensive client intakes, assessments, and ongoing evaluations

Develop, implement, and monitor individualized service or care plans

Provide case management, referrals, and follow-up services

Advocate for clients to access housing, healthcare, financial assistance, education, and other community resources

Maintain accurate, timely, and confidential client records and documentation

Coordinate services with internal staff, external agencies, and community partners

Support clients in goal-setting and progress toward self-sufficiency

Monitor client progress and adjust service plans as needed

Participate in team meetings, case conferences, and required trainings

Ensure compliance with agency policies, ethical standards, and applicable regulations

Demonstrate cultural sensitivity, professionalism, and compassion in all interactions

Job Requirements

Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (required or preferred, as applicable)

Previous experience in case management, social services, or a related setting preferred

Experience working with vulnerable populations is a plus

Strong interpersonal, communication, and advocacy skills

Ability to build trusting relationships with clients from diverse backgrounds

Excellent organizational and documentation skills

Knowledge of community resources and social service systems

Ability to work independently and as part of a multidisciplinary team

Strong problem-solving and time-management abilities
