Job Overview The Mount Sinai Hospital is a 1,134‑bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the diverse needs of our patients. Social Workers collaborate within interprofessional teams to provide direct care, prevention, and community outreach, emphasizing the physical, emotional, and social needs of patients and their care partners.

Responsibilities

Engage patients and care partners to assess psychosocial needs for discharge planning and care management.

Develop interventions, comprehensive care plans, and provide clinical support in accordance with social work practice standards.

Collaborate with providers and the interdisciplinary team to ensure coordinated service delivery.

Document patient encounters and program activities accurately, timely, and consistently with departmental requirements.

Participate in supervision, pursue professional development opportunities, and obtain licensure (LMSW or LCSW) in line with New York State Office of the Professions criteria.

Perform related duties as assigned by the department.

Qualifications

Master’s degree in social work from an accredited institution.

Knowledge of social work theories and techniques.

Six months to one year of related experience in a medical or health care setting.

LMSW or LCSW.

Compensation The salary range for this role is $85,280 - $97,760.36 annually. Actual salaries may vary based on experience, education, and operational need.
