Role Summary:
As a Clinical Liaison (Care Transitions Coordinator), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.
Responsibilities
Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive level opportunities.
Qualifications
Graduate of an approved school of nursing, therapy or social work.
Licensed in the state where you currently practice.
Two (2) years of demonstrated field experience.
Experience with federal, state, and local laws and regulatory guidelines governing Medicare-certified home health and hospice operations.
Basic technology skills, including operation of a mobile device.
Preferred Qualifications
Registered Nurse (RN), Physical Therapist (PT), or Licensed Medical Social Worker (LMSW).
Three (3) years of field experience.
Previous experience in home health or healthcare sales.
Requirements
Valid state driver license.
Maintain automobile liability insurance as required by law.
Dependable, good working condition transportation.
Ability to safely drive in all weather conditions.
