is seeking Registered Nurses to join our team. The RN reports directly to the Director of Nursing. Sign‑on bonus:
$1500 paid quarterly for up to 5 years . Shift: 7 am–7 pm every other weekend and 7 pm–7 am every other weekend required. PRN shifts available. Purpose
The RN monitors the health and wellbeing of residents, provides leadership support, and assists with resident care and services while maintaining clinical quality assurance. Responsibilities
Administer medication care. Communicate with resident support services and families. Maintain clinical quality assurance. Lead, demonstrate, and support all wellness components of Phoenix programs. Identify resident needs through assessment/Personalized Service Plan (PSP) process. Ensure follow‑through and documentation for changes in clinical care needs. Assist the Wellness Director with completing monthly wellness visits and PSP documentation. Inform the Wellness Director and/or Executive Director of medication or service level changes that may affect tier level or potential move‑out. Obtain resident weights and vital signs monthly. Provide direct resident care as needed. Requirements
Must be a Registered Nurse. Minimum one year of experience working with elders or disabled individuals (preferred). Demonstrated leadership capabilities. Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing (preferred). Ability to work weekends, evenings, and flexible hours to meet peak service delivery times. Understanding of infection control procedures, Personalized Service Plans, advanced directives, and end‑of‑life care. Proficiency in MS Office and state audit compliance tools. Strong time‑management, documentation, and grammar skills. Basic knowledge of risk management principles. Benefits
