Licensed Practical Nurse, LPN – Home Visits

Responsibilities
Make home visits to clients within designated territories.
Administer medications, perform wound care, treatments, and procedures.
Monitor client conditions and report changes to the Clinical Manager and Client Services Manager.
Execute physicians’ orders and document them properly.
Conduct client assessments as needed.
Coordinate case management.
Accurately document observations, interventions, and evaluations using electronic medical records on a touch‑pad tablet.
Qualifications
Current licensed Practical Nurse in Nevada.
Minimum one year recent verifiable experience as an LPN.
Graduation from an accredited nursing program, confirmed by transcript or diploma.
Home care experience strongly preferred.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; EMR experience preferred.
Willingness to travel to assigned cases.
