Sr. Staff Nurse - NFP Program Home Visitor RN, BSN Veterans in Healthcare – Lubbock, TX

Job Overview Provide professional nursing care as part of the School of Nursing (SON) Nurse Family Partnership (NFP) Program under the Larry Combest Wellness Center. This role involves home visits, program coordination, and clinical support in a community setting.

Responsibilities

Work day, evening, and weekend hours as required.

Act within the nurse’s legal scope of practice and organization policies.

Assume responsibility for the quality of nursing care provided to patients and families.

Determine physical and mental health status; analyze assessment data to identify problems and formulate goals & outcomes.

Evaluate and report patient outcomes and responses to therapeutic interventions.

Develop, implement, and evaluate teaching plans for patients and families to promote health.

Implement measures to promote quality and safe environment.

Communicate patient data using Electronic Health Records (EHR).

Plan, coordinate, monitor, and evaluate educational programs within the NFP Program.

Provide comprehensive community health nursing services to clients and families.

Maintain a caseload of at least 25 enrolled clients.

Adhere to NFP program guidelines and participate in ongoing training.

Facilitate outcome and community education activities to support the NFP program.

Work closely with the NFP Nurse Supervisor and Data Coordinator to monitor program outcomes.

Develop and maintain therapeutic relationships with clients to assist them in achieving self‑sufficiency.

Practice in non‑traditional settings, including clients’ homes; make professional judgments and collaborate with supervisors to resolve delivery needs.

Drive to client homes and events using personal transportation; maintain vehicle inspections, auto insurance, and valid driver’s license.

Occasional Duties

Attend community partner meetings.

Assist in marketing the NFP Program to the community.

Assist the program data coordinator as needed.

Travel outside Lubbock County, including overnight stays, when required.

Required Qualifications Graduate from an approved accredited nursing program; current RN license in the State of Texas or ability to transfer license to Texas. Minimum of two years of Staff Nurse experience in a specialty area.

Preferred Qualifications

Community experience in maternal‑child health.

Experience conducting home visits.

BLS certification.

Associate’s degree or higher in a related field.

Pay Pay Grade Minimum 21.28, Pay Grade Maximum 47.67.

Travel and Shift Travel required up to 75%; schedule includes day, evening, and weekend time.
