Support patient recovery and ability to stay at home, enabling quality time with loved ones.
Empower patients/families to reach healthcare goals by educating on disease management.
Case‑manage and provide clinical oversight to interdisciplinary team.
Qualifications
RN license in the state you work.
Two years as a registered nurse; home health experience preferred.
Current driver’s license.
Current CPR & BCLS certification.
On call for Neenah agency from 5pm to 8am; 7 days on and 7 days off; salaried position.
