Our rapidly growing home-based primary and palliative care program serves a panel of 600+ patients and continues to expand due to strong community demand. This is an excellent opportunity to join a stable, mission-driven team with experienced clinicians and strong medical leadership.
Position OverviewWe are seeking an experienced Nurse Practitioner or Physician Assistant to provide home-based primary care, palliative care, and limited hospice support for adult and geriatric patients.
This role offers a varied and flexible schedule , including:
1 day per week in a primary care clinic
Home and assisted living visits for primary and palliative care
Periodic hospice support , including inpatient hospice rounding
Remote/telehealth days incorporated into the schedule
Providers have significant autonomy to design their daily schedules based on patient needs.
Locations & CoverageFargo, ND
Provide comprehensive primary care and symptom management for adult and geriatric patients
Conduct home visits and visits in assisted living or long-term care settings
Deliver palliative-focused care with an emphasis on quality of life
Collaborate closely with physicians, NPs, RNs, social workers, and interdisciplinary team members
Participate in weekly in-office team meetings (limited additional office time required)
Average 6 8 patients per day in the field
Utilize AI-supported documentation tools to streamline charting
Full-time preferred (open to hours/week )
Providers may adjust start times (e.g., early morning visits) based on patient needs
Mix of in-person and remote work
Minimum of 2 years of experience as a Nurse Practitioner or Physician Assistant
Background in primary care , ideally with a geriatric patient population
Palliative care experience strongly considered
Home-based care experience highly preferred (must be comfortable working independently in the field)
Licensed or eligible for licensure in North Dakota and Minnesota
DEA license required
Base salary range: $115,000 $140,000+ , depending on experience
Bonus potential available
Mileage reimbursement: $0.54 per mile
