Optometrist

to support a Federally supported client in

Astoria, OR (Zip Code 97103) .

Optometry services are required on a weekly basis to assist students (populations range from 100 to over 300) in maintaining optimal eye health and visual function as part of comprehensive health services provided at the Center. These services support the Health and Wellness program by ensuring students develop the visual readiness, awareness, and preventive care habits that promote safety, learning, and employability.

LOCATION Our client provides care at a Wellness Center servicing at-risk youth on a residential campus designed to prepare them for meaningful employment. Students range in age from

16 through 24

and receive coordinated medical, behavioral, and wellness services at our location in

Astoria, Oregon (Zip Code 97103) .

PERIOD OF PERFORMANCE 10 hours per week

or

2 days out of the normal work week. The facility operates between the hours of

8:00 a.m. and 6:00 p.m., Monday–Friday. No weekends required.

ROLES/RESPONSIBILITIES

Perform comprehensive eye examinations, including visual acuity testing and refraction

Evaluate ocular health and diagnose eye diseases and visual disorders

Provide corrective prescriptions for eyeglasses or contact lenses

Conduct vision screenings as part of student entrance assessments

Identify and manage conditions such as refractive errors, strabismus, amblyopia, and ocular injuries

Coordinate referrals to ophthalmology or specialty eye care when advanced treatment is required

Participate in health education initiatives related to eye safety, vision hygiene, and preventive care

Maintain accurate documentation in accordance with federal and facility standards

Collaborate with nursing, primary care, and mental health teams to support coordinated care

Provide follow-up care and monitor treatment progress as clinically indicated

QUALIFICATIONS OF CONTRACTOR PERSONNEL

Doctor of Optometry (OD) degree from an accredited institution

Ability to practice independently

Active and unrestricted

Oregon Optometry license

Malpractice/professional liability insurance and general liability insurance

Certification in

CPR/AED

Experience working with adolescent or young adult populations preferred
