Care Manager Assistant- Cantonese/Mandarin Speaking (Brooklyn, NY) (Education)
Job Requisition The Care Manager Assistant provides face-to-face visits, ongoing support and care coordination to members. Responsibilities include face-to-face visits, communication and follow up with members and reviewing of person centered service plan with member/members family and coordination of covered and non-covered benefits. Fieldwork required. Essential Responsibilities: • Conduct multiple types of mandatory face to face visits based on need• Schedule members due for face-to-face visits• Meets weekly productivity expectations• Completes necessary documentation, scripts, activities and follow ups - ensure all items are addressed at time of visit• Understands and supports company wide goals based on direction• Participate in weekly team meetings and ongoing training• Other duties as identified as a necessary part of employee's role Specific Knowledge, Skills, and Abilities: • Able to travel and conduct home visits• Knowledge of member assessment procedure• Excellent interpersonal skills - ability to speak effectively with members and employees of the agency• Demonstrates the ability to effectively communicate information with members of all ages and socioeconomic levels• Ability to read and interpret documents, understand and execute workflows Required Education, Experience, and Licenses: • The Appropriate background and experience in Care Management and/or MLTC services.• Bilingual preferred (Cantonese/Mandarin). • Bilingual preferred (Spanish).
Pay $29-$31 per hour Work Location: On the road - Brooklyn, NY Monday to Friday
