Reports to: Psychiatric Rehabilitation Specialist/Supervisor.
Shift: Fri/Sat 11pm-7am.
Job purpose:
The Psychiatric Rehab Worker provides supportive services to individuals with mental illness living independently in the community. Psychiatric Rehabilitation focuses on building independent adult living skills to gain and maintain independence and reach personal growth potential. Psych Rehab Workers with lived experience provides peer support services and advocate for participants, shares coping skills, and provides recovery information for participants. The PRW must be willing to travel throughout the community on daily basis, and have a working knowledge of community resources throughout the area of Bucks County.
Essential Functions
- Mobile Work- Need to have the ability to be mobile to meet with program participants independently in the community and at their residences.
- Encounters- Must make the productivity standard of 60% billable encounters with participants during the work week.
- Documentation
- Encounter Notes must be entered into the database within 24 hours of the encounter.
- Individual Recovery Plans, Reviews, and Assessments must be completed and approved before the 90-day deadline.
- Intake Paperwork must be submitted and complete for all new participants.
- Concurrent Reviews assigned must be completed an approved before the deadline provided by the Team Lead.
- Training- Completing Assigned Training, quizzes, and confirmed understanding on how to implement what was learned.
- Disciplinary History- Adherence to Psych Rehab Services and RHD policies including call outs, professionalism, and other program and organizational standards.
Duties and responsibilities:
Work with Participants to Obtain Skills to become Independent and Integrated into their Community:
- Serve as a member of the PRS Team to provide psychiatric rehabilitation services to participants living independently in the community.
- Promotes the principles of mental health recovery.
- Identify rehabilitation goals to support participants with building skills for independent living and community integration activities.
- Support participants to maintain ongoing communication with the many members of their support services to ensure all needs are met. This includes but is not limited to ICM/RC, landlords, doctors, family and friends.
- Share recovery oriented educational materials.
Implement Wellness Recovery Action Plans(WRAP):
- Engage in discussion with participants about WRAPs.
- Facilitate creation and integration of WRAPs as desired by participants.
- Facilitate WRAP groups as needed.
Maintain Compliance with Funding and Licensing Entities Related to Service Delivery:
- Complete Documentation of Quality Service Delivery daily. Required Documentation includes daily notes of billable encounters and nonbillable outreach attempts, Individual Recovery Plans, and Concurrent Review Reports.
- Complete all Intake and Discharge documentation with participants and submit completed work to PRS Supervisor.
- Attend all training as required by regulations and as desired for professional development.
- Attend schedule team meetings.
- Attend all Linkage meetings.
- Participate in Supervision at least twice monthly.
Follow Psych Rehab Services and RHD policies and procedures:
- Obtain productivity standard: 60% of work time is to be spent as billable encounters with PRS.
- Communicate use of work time to accomplish tasks through Outlook, email and calls to the Psych Rehab Specialist Supervisor. Document work time accurately in Lawson payroll database.
Complete all other assigned tasks as designated by the PRS Supervisor.
Qualifications:
Education:
- HS Diploma/GED and 2 years work in human services which must include 1 year of mental health experience.
OR
- Associates Degree and 1 year work experience in mental health direct service.
OR
- CPS Certificate and 1 additional year paid or volunteer work in mental health direct service.
OR
- Bachelor Degree (preferred).
- CPRP (preferred).
Knowledge/Skills:
- Knowledgeable of recovery principles and tools.
- Knowledgeable of psychiatric rehabilitation.
- Knowledge of WRAP.
- Able to communicate in a written or verbal format.
- Able to document daily activities and recovery goals.
- Able to role model recovery and hope.
- Able to hear anecdotal information and reframe in the context of facts and data in an emphatic manner.
- Knowledgeable of Microsoft Office applications (Word, Excel, Outlook).
- Able to manage mobile technology independently.
- Able to support and implement RHD and unit values and beliefs.
Other Requirements/Certifications:
- State police clearance.
- FBI clearance is required.
- Child Abuse Clearance.
Working conditions:
RHD-Psych Rehab Service is a mobile program. This is mobile positions requiring traveling throughout Philadelphia during the work day to meet with participants at multiple locations and providing skills training and community integration with participants which also may include travel.

Additional Special Working Conditions:
Physical requirements:
The Psych Rehab Worker will need to be able to be mobile to travel to participant apartments and have physical capability to do this. Walking and the ability to carry a laptop or other mobile devices, books, binders or other work items may be needed while traveling in the community.
Lifting Requirements:
- Sedentary: exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to left, carry, push, pull or move objects (including self). Most work involves sitting majority of the time.
- Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
- Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
- Heavy: exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
- Very Heavy: exerting over 100 pounds of force occasionally, and/or move 50 pounds of force constantly to move objects.
