Case Management Supervisor

Job Summary Assists the Department Director in organizing, managing and overseeing the operation of the department. Maintains accountability for the coordination of case management activities during business hours. Directs and supervises the case management staff in the delivery of case management services. Directs and delivers comprehensive case management services to patients in a safe and proficient manner in accordance with individual patient needs.
Major Responsibilities
Provides direction and supervision for the Case Management Department:
Monitors case management activity to assure effective interface between all members of the healthcare team. Acts as liaison to the healthcare team and other system facilities to ensure effective communication and cooperation.
Assists director in evaluation of case management process for potential redesign in response to changing needs of systems, medical staff and patient population.
Maintains current knowledge of Community Resources, Discharge Planning Process, Interqual criteria, medical necessity, and reimbursement and denial structures.
Demonstrates effective communication and conflict resolution skills. Able to effectively intervene in difficult situations in a professional manner.
Provides oversight and direction of department operations:
Assists the Director in ensuring timeliness and accuracy of concurrent reviews, and intervenes in situations where patients may not meet medical necessity and monitors processes to manage and respond to denials.
Able to assist department with workload changes as appropriate and necessary to meet patient and facility demands.
Maintains communication of department and area initiatives through regular staff meetings.
Assists director with employee performance evaluations, counseling, and interviewing processes.
Functions on behalf of the Director in his/her absence.
General Duties:
Maintains current knowledge of resources available within the community, maintains supply of resource materials for distribution.
Prepares and presents in‑service and training programs as requested.
Collaborates with interdisciplinary care team, patient and family in the assessment, coordination, and implementation of discharge planning needs, delivery of post‑discharge planning services and transition of the patient from the hospital to the discharge setting.
Demonstrates understanding of HCA’s and “Patients First” safety initiative by strict compliance with all safety protocols and procedures.
Assumes other duties and responsibilities as assigned.
Education & Experience
Graduate of an accredited School of Nursing – Required
Bachelor's Degree or accredited school of nursing – Preferred
2+ years experience in an acute care setting – Preferred
2+ years experience in acute care hospital case management – Preferred
Licensure, Certifications, Training
Currently licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.
Full‑time, part‑time or PRN colleagues with a compact license from a state other than Texas must:
Apply for a Texas RN license within 60 days from hire.
Obtain a current TX RN license within 6 months of application; required for all departments.
