In-Home Transition Specialist – Home & Community-Based Waiver (HCB) (Multilingual Required)
Base pay: $22.00/hr – $25.00/hr

Why This Role Matters The In‑Home Transition Specialist acts as key liaison between the participant, power of attorney/guardian, caregiver, and care coordinator to keep HCB waiver services safe, person‑centered, and compliant. The role focuses on matching and onboarding the right caregivers, caregiver training, in‑home assessments, and incident follow‑through so people can remain safely in their homes and communities instead of institutional settings.
What You Will Do
Serve as the primary contact and relationship bridge for the participant, power of attorney/guardian, unpaid caregivers, and the Golden Years ADHC care coordinator.
Explain the plan of care, service expectations, and participant rights in clear, respectful language and help resolve routine questions or concerns.
Support person‑centered planning by ensuring participant goals, preferences, and safety needs are reflected in the in‑home plan of care.
Assist in identifying and recommending the right caregiver match for each participant based on needs, preferences, skills, schedule, and home environment.
Lead or support the caregiver transition into the home, including first‑day introductions, review of expectations, and warm hand‑off from the care coordinator.
Provide structured, waiver‑aligned caregiver training and coaching on authorized tasks (personal care, safety, infection control, fall prevention, documentation, boundaries, and communication).
Reinforce caregiver understanding of incident reporting expectations, confidentiality, and respect for participant choice and dignity.
Monitor caregiver placement and performance during the early phase of assignment and communicate needed changes or additional training to the care coordinator and leadership.
Conduct scheduled home visits to review, validate, and document that services match the HCB waiver plan of care, including tasks, hours, and safety needs.
Complete phone‑based assessments—both announced and unannounced—to verify caregiver presence, service delivery, and participant wellbeing.
Observe the home environment for risks and strengths and document findings using Golden Years tools and Medicaid‑aligned standards.
Educate caregivers and participants on what constitutes an incident (falls, medication errors, suspected abuse/neglect, serious injuries, etc.) and how to report it.
Receive and document incident information, ensure immediate protection steps are taken, and follow Golden Years and Medicaid/HCB incident reporting procedures and timelines.
Track follow‑up actions, communicate with the care coordinator and leadership, and support corrective actions to prevent recurrence.
Schedule & Work Environment
Base: Lexington, Kentucky office; daily and weekly travel within approximately a two‑hour radius to participant homes and community locations.
Status: Full‑time (typical weekday daytime hours) with occasional flexibility for early evening calls or visits based on participant and caregiver needs.
Environment: Mix of office work (calls, documentation, coordination) and field work (home visits, caregiver transition, training, and follow‑up), with occasional collaboration with the adult day center team.
Pay, Benefits & Growth
Starting pay: $22.00–$25.00 per hour, based on experience, certifications, and skills.
All new team members are on a 90‑day probationary period to confirm reliability, cultural alignment, and role fit before transitioning to regular employment status.
Paid training and “hip‑to‑hip” onboarding with experienced team members in HCB waiver expectations, caregiver selection/onboarding, training methods, incident reporting, and Golden Years policies.
Eligibility for benefits (health, PTO, holidays, mileage reimbursement, etc., consistent with Golden Years policies).
6‑month review and annual performance reviews with clear KPIs tied to documentation quality, caregiver match and training completion, timely incident reporting, and participant/caregiver feedback.
What We’re Looking For (Must‑Haves)
Experience in home and community‑based services, caregiving, case management, life enrichment, or similar roles with Medicaid waiver or HCBS experience strongly preferred.
Multilingual preferred (Arabic, Swahili, French, Spanish, etc.).
Strong communication skills and ability to build trust and maintain professional boundaries with participants, caregivers, and external partners.
Comfort conducting frequent in‑home visits and field travel within a two‑hour radius of the Lexington office, plus completing phone assessments (announced and unannounced).
Demonstrated ability to coach, train, and support caregivers, including giving clear feedback and reinforcing expectations.
Solid documentation skills and attention to detail for plans of care, visit notes, training records, and incident reports.
Reliable transportation, valid driver’s license, and current auto insurance for daily/weekly travel within the service area.

Experience with Kentucky HCB Waiver programs, participant‑directed services, or other HCBS waivers.

Familiarity with electronic visit verification (EVV) and basic EMR/waiver documentation systems.
Pre‑Employment Requirements
Successful completion of a criminal background check.
Drug screening and TB test.
Ability to complete required onboarding steps within the requested timeframe after a conditional offer is made.
