Position Overview
A-Line Staffing is looking for Nurse Case Managers across multiple counties! The Nurse Case Manager II utilizes a collaborative, clinical approach to assess, plan, facilitate, and advocate for services that meet members’ health and benefit plan needs. This role supports elderly individuals and persons with disabilities who are approved for in-home care or nursing facility care, ensuring quality, cost-effective outcomes.
This is a field-based position requiring 50–75% travel to members’ homes. The Case Manager will work remotely from home between visits and must be comfortable working independently.
Key Responsibilities
Conduct comprehensive clinical assessments using standardized tools, data review, and eligibility criteria
Evaluate member benefit plans and coordinate internal and external programs and services
Develop and implement case management plans in compliance with regulatory, accreditation, and company standards
Apply clinical guidelines, policies, and procedures to ensure appropriate benefit administration
Utilize case management and quality management processes to support optimal outcomes
Collaborate virtually with interdisciplinary teams using clinical systems and communication tools
Required Qualifications
Active, unrestricted Illinois license as one of the following:
Registered Nurse (RN)
Licensed Clinical Social Worker (LCSW)
Licensed Clinical Professional Counselor (LCPC)
Illinois resident living in one of the target counties listed above
Experience in case management
Valid driver’s license and reliable vehicle
Ability to travel to member homes up to 50–75% of the time
Strong organizational skills with the ability to work independently from home
Preferred Experience
3–5 years of clinical practice experience
2–3 years of care management, discharge planning, or home health coordination experience
Experience working with elderly populations and individuals with disabilities
Certified Case Manager (CCM) preferred
Skills & Competencies
Strong analytical and problem-solving abilities
Excellent communication, interpersonal, and organizational skills
Confidence working independently in a remote/field environment
Proficiency navigating multiple computer systems
Working knowledge of MS Word, Excel, Outlook, and PowerPoint
Work Environment
Field-based with home office between visits
Independent, self-directed role with virtual collaboration
Travel required within assigned county/ZIP area
