In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot and make sound decisions related to patient dialysis treatment and may coordinate care with SNF staff. Responsibilities
Direct Patient Care: independently demonstrate best practice techniques and deliver optimal dialysis care; supervise Dialysis Technicians who support care delivery. Patient education: conduct impactful and meaningful education to patients and their families in the SNF setting. Autonomy and Critical Thinking: apply core nursing skills to assess, diagnose and implement care plans; manage multiple tasks to meet service needs. Collaborate with cross-functional resources across the organization as needed. Schedule & Travel
Primary work schedule will be 3-4 days a week with some travel to other SNF clinics within the region (depending on business/patient needs). Alternating Saturdays may be required. The RN will be based at Lincoln Hills and may travel to additional SNF clinics as needed. Qualifications
Current RN license in the state of practice; minimum of 12 months of RN experience. Associates Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred; three-year diploma may be substituted for nursing degree. Basic computer skills and proficiency in MS Word and Outlook. Preferred/Nice-to-Haves
Dialysis experience (previous chronic hemodialysis or acute dialysis) preferred but not required. Training/Teaching/Healthcare Education background. Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN). Training & Development
Training includes a combination of classroom and hands-on learning through DaVita’s paid training programs. Training may occur in a facility or training clinic with a class of new teammates other than the assigned home clinic. Training hours may vary; typically 32-40 hours per week during training. May be asked to support other local SNF sites. What We’ll Provide
