Responsibilities
Responsible for admissions on assigned units and review all new admissions documentation within the prescribed time frames.
Review all new admission baseline care plans within 24 hours and update comprehensive care plans with changes as necessary.
Ensure resident consults, appointments and escort arrangements are in place and labs, diagnostic studies follow up and documentation are done.
Complete all quarterly assessments.
Attend all Care Plan meetings for residents on assigned units.
Complete residents MDS interviews.
Communicate all changes in medications and residents’ condition to family members.
Attend all family/resident related meetings.
Attend morning meetings with the IDT.
Work collaboratively with assigned unit supervisors on all three shifts.
Provide guidance and training to all unit staff as needed.
Educate residents and families.
Contribute to staff evaluation and counseling.
Minimum Qualifications
Reassignment: Current New York State Department of Health Veterans Home at Montrose employee with permanent or contingent permanent status as a Registered Nurse 2 (Geriatric).
Transfers: Current New York State employee with one or more years of permanent, contingent‑permanent, or 55 b/c service in a position allocated to Grade 18 or above, and eligible for transfer under Section 70.1 to Registered Nurse 2 (Geriatric).
Promotion Provisional Appointment: In the absence of an eligible list with three or more candidates willing to accept a position, a temporary provisional appointment may be considered. Candidates must meet one of the following minimum qualifications: possession of a license and current registration as a registered professional nurse in New York State; and two years of post‑licensure clinical nursing experience working with a geriatric population.
Open Competitive HELPS qualifications: Possession of a license and current registration as a registered professional nurse in New York State; and two years of post‑licensure clinical nursing experience working with a geriatric population.
Substitution
A bachelor's degree in nursing may substitute for one year of general post‑licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post‑licensure clinical nursing experience.
Preferred Qualifications
Experience with geriatric population.
Additional Comments
One contingent permanent, competitive, full‑time appointment, or non‑competitive full‑time appointment via HELPS.
Transfer candidates may be asked for their Social Security Number to verify eligibility.
Degrees awarded from educational institutions outside of the United States or its territories must be independently verified for equivalency.
Additional compensation may apply based on shift assignment: 7% shift differential for evening shift; 10% shift differential for night shift.
