Home Healthcare License Practical Nurse (LPN) . Our Home Health LPNs deliver high-quality nursing care to patients in their homes under the supervision of a registered nurse (RN). This role involves assessing patient needs, implementing care plans, and assisting with daily activities to enhance patient well-being.
Overview We are a home healthcare provider serving Colorado with comprehensive, skilled and non-skilled services for homebound clients. We strive to help clients improve their lives while remaining in the comfort of their homes.

Responsibilities
Provides direct patient care as defined in the state Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, reporting identified needs to the registered nurse and/or clinical supervisor.
Uses and/or prepares equipment and supplies effectively and adheres to aseptic technique.
Participates in personal and professional growth and development.
Performs other duties as assigned by the registered nurse.
Qualifications
Graduate of an accredited practical nurse or vocational nursing program
Three (3) years of nursing experience; community/home health or medical/surgical experience preferred
Currently licensed as an LPN/LVN in the state
Valid CPR certification
Complies with accepted professional standards and practice
Good verbal and written communication and organizational skills
Job Type
Full-time, Part-time
Schedule Monday to Friday; weekends as needed
Experience in Home Health 1 year (Preferred)
