REGISTERED NURSE - OR - LCSW - MANAGER CASE MANAGEMENT
Progeny HR Solutions LLC is seeking a highly qualified and experienced Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) to join our team as a Manager of Case Management in Savannah, GA.
Responsibilities
Oversee and manage the case management process for clients, ensuring timely and effective communication with all parties involved.
Conduct initial assessments and develop individualized care plans for clients.
Collaborate with healthcare providers and other professionals to coordinate and facilitate the delivery of quality care.
Monitor and evaluate the progress of clients and make necessary adjustments to care plans.
Provide support and guidance to case managers and assist in resolving any issues that may arise.
Ensure compliance with all relevant laws, regulations, and company policies.
Maintain accurate and up-to-date records and documentation.
Participate in team meetings and provide input to improve processes and procedures.
Act as a liaison between clients, families, and healthcare providers to ensure a smooth and efficient case management process.
Requirements
Current and valid RN or LCSW license in the state of Georgia.
Minimum of 3–5 years of experience in case management, preferably in a healthcare setting.
Strong leadership and communication skills.
Excellent organizational and time‑management abilities.
Ability to work independently and as part of a team.
Proficient in computer skills and electronic medical records.
Knowledge of relevant laws, regulations, and policies related to case management.
Compassionate and empathetic approach to working with clients and their families.
Willingness to travel within the Savannah area as needed.
