Pay
$70,000 – $78,000 annually

Responsibilities
Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines as indicated or document rationale for not doing so.
Promote patient health and independence through teaching and appropriate rehabilitative measures.
Assist patients in learning self‑care techniques.
Supervise LPNs and HHAs.
Complete documentation in a timely and accurate manner.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform on‑call responsibilities and services to patients/families as assigned.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
Qualifications
One (1+) year of clinical experience as a Registered Nurse (RN); if less than 1 year, the candidate must be approved by VP Clinical.
Current RN license specific to the state(s) where the role is assigned.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.
If less than 6 months clinical experience as an RN, the candidate must participate in the RN Intern program.
